For New Patients!
If you would like to help us speed up the process of your first appointment in our office, print off your Registration Form and Medical History Form at home, fill them out before you come, and then bring them to your appointment!  Also, if you are transferring to our office from another dentist, print out and fill out the “Records Request” and mail or give it to your former dentist so that we can obtain your current radiographs, records, etc. from them.
